Create a Parent Initiated Form (PIF), including Workflow and Parent experience.

A Parent Initiated Form (PIF) is one that is designed to be started by the Parent of a Student. Sample use-cases include Enrolment Forms or Medical Details Update Forms.

Parents receive an email notification after they have submitted a PIF. This is sent to the email address linked to their account and includes the Title of the PIF they submitted and the Student it related to. They will also see it has been completed on their MyForms page and can view the information they submitted from there, too.

The basic Workflow for a PIF is Create and Complete, but you can add Approval and Staff notification steps, too.

Steps:

Click Form Templates from the LHS menu and click New Template. Select the type Parent Initiated Form for Student.

Enter the Form name and Description of the Form Template. You can also:

  1. Allocate the Template to a pre-existing Category

  2. Require the Parent to graphically sign the Form (default is No)

  3. Add any new Students created via the Form to a pre-existing Student List

  4. Display to Parents so they can initiate the Form from their My Forms page (available when they are logged in)

    • You will need to set this to Yes for Enrolment Forms or the like.

When finished, click Add New Form.

 

The Form Template view page shows you the configuration for the new Template. Note the Create Form URL. You can have this on your web site, or send it to a parent in an email or an EdSmart Broadcast. When they click on the link they will be asked to create a sign in to EdSmart (or create an account if they are new). Afterwards, they can enter their Student's details, complete and submit the Form.
