School Form for Student and Parent Response
EdSmart has a template type called School Form for Student, with Parent Response that is created by a User sent to Parent referencing a Student. Both Student and the Parent can respond to the form and submit it back to the School.
Unlike Slips, this template type does not use Student Lists. You can add up to 10 recipients per Form when you create it from the Template. Recipient are added manually.
The template design will be similar to the Slips, there will be a School and Parent section from which fields can be used as per your requirements. The workflows behave differently based on how they are set up. You can find more information on it in the below article. We recommend going through this KB to make use of all the possible scenarios with this template type
Different Workflows for School Form for Student and Parent Response
Student/Parent - Information
User creates the Form and clicks on the Send to Student for Response, a notification will be sent to the Student after a few minutes, as shown below
Student opens the Form in the portal
Student answers all the questions and clicks on the Student Submit Form
After a few minutes, the Form will be sent to the Parent
Parents will get an email notification similar to the Slip and they can access it directly from the link or from the portal.
Parent will see Student’s answers and updates the Form if necessary
Parent submits the Form

Staff will need to follow the following instructions:
School Staff
Click the edit button at the bottom of the Form. This will allow you to begin entering your responses.
When you complete your part of the responses, click the save button at the bottom of the Form. If you do not complete your responses in one sitting and wish to come back to this Form, click the save button and the Form will save and remain in the draft format until you return to it.
You will then be offered a draft to review your response. If you wish to make a change/addition to your responses, please click the edit button and make your changes/additions. If you are satisfied with your responses please click "send to next person" or complete the Form if you are the last person in the workflow.
