Student transitioning/departing school example form

Fields used in the Activity Information section of the template:

• Long Description Field for Transition Letter - This field is used to enter the letter from the Principal giving information about a Student transitioning/departing the school.

Fields used in the Parent Information section of the template:

• Option List - For parents to select Yes or No for the question 'My child will be leaving College in 2020'
• Text Field - For parents to enter the name of the school their child is moving to
• Check Box List - For parents to select any of the following options:
  • We are undecided at this time
  • We request a meeting to help with our decision making (we would like to attend morning tea on 11 June)
• Parent Consent - For parents to acknowledge the above information they have provided is correct
