EdSmart has a template type called School Form for Student, with Parent Response that is created by a User sent to Parent referencing a Student. Both Student and the Parent can respond to the form and submit it back to the School.
Unlike Slips, this template type does not use Student Lists. You can add up to 10 recipients per Form when you create it from the Template. Recipient are added manually.

The template design will be similar to the Slips, there will be a School and Parent section from which fields can be used as per your requirements.
