  1. Edit the Form Templates you wish to be Children of this Packet (Secondary Forms)
  2. Allocate them to the Parent. Note, that a Template can be the Child Form (Secondary Form) of more than one Parent Form (Primary Form).

To assign the order of the Child (Secondary) Forms:

  1. Navigate back to the Parent (Primary) Forms:
  2. In the Section In this Packet enter numbers in ascending order based on the order the Forms should be displayed.

When a Form is created from the Parent Template, it will be shown with a tabbed view. The first tab is the Parent Template (Primary Form), and the others are the Children (Secondary Forms). These will be displayed in the order as per the display order assigned in the Template's setting.


When a Parent/Caregiver is submitting a Form Packet, there's the option to 'Save Draft' at the bottom of the page.

When selecting this option, it closes the current instance and the Parent/Caregiver is redirected back to their 'My Forms' screen, leaving their packet in 'Draft'. This means they can return to it at a later stage.


For a Parent/Caregiver to complete all the child Forms for the relevant EdSmart Student, the Parent/Caregiver can select each child Form at the top of the packet page, by clicking on each tab individually.

If a Parent/Caregiver selects 'Parent Submit Form' and they have missed mandatory fields anywhere (marked by a red *), the relevant child Form tab will be highlighted in red. They will need to click on this child Form, navigate to the missed field, and fill it out with relevant details.

This will occur for each child Form with Mandatory Fields incomplete or, if relevant, finalised consent not given for each child Form.
